Life Care Plans


Continuing Care Concepts provides Life Care Planning services for individuals who have suffered a catastrophic disability. The Life Care Plan is a tool used for the purpose of estimating medical and non-medical needs of a person with a life time disability. It projects the cost of goods and services over the individual's estimated life span.

The Life Care Plan is developed using a team approach and includes a comprehensive assessment, thorough review of the medical records, and interviews with all involved health care professionals. Included also is a comprehensive evaluation of the individual's potential for independent functioning. It is written to the specific needs of the individual. The costs included in the plan are based on today's dollars. If indicated, a complete vocational assessment and plan is obtained from a vocational rehabilitation specialist.
