Level of Care Assessment

For those clients who cannot live safely at home, Continuing Care Concepts offers assessments to determine the appropriate placement setting whether it be congregate or assisted living, residential care (board-and-care) skilled nursing or a dedicated dementia facility.

Placement Assistance

Direct Placement Assistance is available for those clients needing congregate living, skilled nursing, or a dedicated dementia facility. Referral will be made to an appropriate placement agency for those clients who need residential care.

Discharge Planning

Discharge planning services are available to those clients able to leave to return home from either residential care or a skilled nursing facility.

Nursing Assessments For Conservatorships

A comprehensive assessment of a client's physical and functional abilities to provide for his or her own basic care is made if there is an indication that an alternative decision maker is needed. A mini-mental status exam is included and referral for further cognitive testing is made through the primary physician if there is any question of the clients cognitive status. This documentation is presented to the court as part of the conservatorship process.

An hourly fee is charged with the exception of the initial assessment which is charged at a flat rate. The services provided are determined by need and agreed upon by Continuing Care Concepts and the client or responsible party.

We accept referrals from physicians, discharge planners, attorneys, trust departments, community organizations, and public agencies as well as from individuals and family members.
